Our Occupational Therapy services are tailored to address the unique needs of each individual, providing evidence-based support through key life transitions to foster long-term success and personal growth.
Transition Services
We help students and young adults navigate the path from school to greater independence. Our team provides individualized support to build life skills, confidence, and self-efficacy for meaningful participation in home, work, and community settings.
We are in the process of becoming an approved Home and Community-Based Services (HCBS) provider through DDDS, with services available to both waiver-funded and private-pay participants.
Transition Consultations Services
Our Transition Consultation Services provide expert guidance and practical strategies to support individuals, families, and educational teams through every stage of the transition process. These consultations complement our Transition Services, offering a collaborative space to identify goals, build essential life skills, and create personalized plans that promote independence, self-efficacy, and successful community participation.
